Do I include a website if it no longer exists or has changed in a fundamental way?

Yes. If a site's operations have changed—whether it has been shut down, has changed ownership, or otherwise has ceased to function as it did at the time of controversy—include that information in a parenthetical. For instance, you might write the name of a shut-down site as “NameofSite (defunct).”
