These fields identify the attorneys for each side of a legal threat or lawsuit. If there are multiple parties on one side, you may note which attorneys represent which parties on that side. For example: “Attorney Smith (for Plaintiff Jones), Attorney Darrow (for Plaintiff Small).” This information typically will be available on court documents and dockets related to the case.
If a party is not represented by an attorney, you should write in the counsel field that the party is "pro se." For example: "Plaintiff Jones (pro se)."February 11, 2012
